This study analyzed the clinical data of 100 cases of pregnant women in our hospital from October to April 2013 in our hospital. The clinical data were analyzed, and the effect of clinical nursing pathway combined with -2014 was discussed.
1 materials and methods
1.1 average data using a random cluster sampling method, selected in our hospital in April 2013 - 2014 from October in 100 cases of pregnant women, all antenatal fetal heart monitoring showed that the fetus has good oxygen reserves. All patients signed the informed consent. There will be complications and postoperative complications of maternal pregnancy excluded in [1]. According to the random number table method, the pregnant women were divided into two groups: the experiment group and control group, 50 cases in each. Maternal age of the study group was 22~34 years old, average (28.3 + 10.1) years old. In the times, 30 cases of primipara, 20 cases were multipara; cesarean section in the producing reason, 23 cases of maternal uterine scar, 16 cases of pregnant women for fetal abnormalities, six cases of maternal social factors cesarean section, 5 cases of pregnant women for umbilical cord around the neck and less amniotic fluid; in cultural degree, 5 cases of pregnant women for primary school, 32 cases of pregnant women for middle school, 13 cases of maternal for college and above. The control group was 21~36 years old, average (29.3 + 10) years old. In the times, 27 cases of primipara, 23 patients were multipara; cesarean section in the producing reason aspects, 25 cases of maternal uterine scar, 15 cases of pregnant women for fetal abnormalities, 3 cases of pregnant women for the social factors of cesarean section, 7 cases of pregnant women for umbilical cord around the neck and less amniotic fluid; in cultural degree, 8 cases of pregnant women for primary, 28 cases of pregnant women for secondary school, 14 cases of maternal for college and above. There was no significant difference in the baseline data between the two groups (P>0.05), which was comparable to that of the group. This study is approved by the hospital ethics committee.
1.2 method
1.2.1 control group, the control group was received routine nursing, the specific operation is: mothers were admitted to the hospital after the first time from the main class on the history of midwives inquiry, the examination results and the birth process were observed carefully, the effective nursing records produced, and details of the hospital environment for pregnant women, by other midwives are good preoperative preparation, will introduce the related knowledge of patient to maternal, maternal escorted to the operation room, receiving the baby, the main class baby area nurses after cesarean section on the basis of routine nursing and the level of regular rounds, 1 times a day, careful observation and nursing care on maternal illness and wounds, will be introduced to the relevant health knowledge maternal and careful writing of nursing records, treatment, nursing classes, class class nurses wash the baby according to the provisions of each class of their duties seriously carry out good, complete treatment, basic nursing care, baby care etc..
1.2.2 research group to study group of maternal clinical nursing path combined with continuing care. (1) clinical nursing pathway. In junior high in the hospital nurses and physicians under the guidance of the clinical nursing path design, design process strictly according to colleges and universities, economic, high-quality principle, then form, pre on clinical nurses were the training of clinical nursing path, make it in daily nursing work strictly in accordance with the operating path execution. Prior to surgery the day of admission for perfect preoperative preparation, will focus on psychological intervention, through maternal effective psychological counseling, promote the pain feeling of significant loss, tolerance and to greatly enhance treatment compliance and effective guidance caregivers, the maternity nursing assistant and psychologically maternal care fully. On the same day, the pregnant women were sent to the operation room before the positive encouragement, so that it can effectively relieve the psychological stress. After operation of nursing staff were careful guidance, the maternal vital signs were closely observed and basic nursing on pregnant women at the same time. Maternal discharge of a good guide to hospital discharge, and to introduce the contents of the continuation of the continuation of the health, will be issued to the health of the mother's hands, will contact the book set up. The detailed items are included in all of the items, and the project is completed by the program, and the clinical pathway is evaluated by the person specially assigned to the project. (2) continuity of care. The postoperative assessment of the table to develop, the maternal discharge at the time of delivery to the maternal, the contents of the incision, mental state, sleep, etc., and the post operation notes attached to the following. Regular telephone follow-up, 1 times a day, to the recovery of the maternal effective assessment of the maternal question to timely feedback, and its effective rehabilitation guidance, to encourage their active activities, etc..
1.3 observation index is observed and recorded in detail hospitalization days, cost of hospitalization, related knowledge awareness and maternal satisfaction and homemade related knowledge questionnaire score in 0-100 points. 70 points = no master; 70 to 89 points = master the basic; is more than or equal to 90 = Master [2]. And self-made satisfaction questionnaire, consists of very satisfied, satisfied, not satisfied with the three options, the control group of corresponding data recorded in detail.
1.4 statistical analysis of all data using SPSS 20 software for statistical processing, measurement data to (x- + s) said, using t test, count data to rate, the use of word 2 test, P<0.05 for the difference has statistical significance.
2 Results
Two the average length of hospital stay, hospitalization expenses and health education test scores of 2.1 groups were shorter than those in control group (P<0.05), the cost of hospitalization was lower than that of control group (P<0.05), and health education test scores were significantly higher than those in control group (P<0.05). See Table 1
Two 2.2 groups of maternal satisfaction with clinical nursing work, the satisfaction of clinical nursing work of the study group was significantly higher than that of the control group 74% (47/50) (P<0.05) (37/50). See Table 2
3 discussion
Clinical nursing pathway is a clinical nursing staff in the process of physical and mental overall care of patients in the course of the design has been a good care model, in the daily work of clinical nursing staff effective assessment and feedback in patients with existing or potential nursing problems, the improvement of their own care, so as to promote the quality of care significantly improved. It belongs to a kind of active nursing mode, the main purpose is to promote the time of hospitalization of patients with significantly shortened and the cost of hospitalization has been greatly reduced, can only to the doctor ordered to carry out blindly the phenomenon of passive implementation to avoid, so as to effectively reflect the clinical nursing staff's personal value. In short, the clinical nursing path combined with the continuity of care in the planning of cesarean section of the nursing effect is remarkable, it is worth promoting.
